The reports and plans (1977-1983) include community health service written plans submitted by the applicants detailing development, implementation, coordination, and operation of community health services that would meet the priority needs of the community. An application accompanied the plan. Each applicant also had to submit an annual report of the activities of the board of health. The reports and plans were actually produced between 1977 and 1983 but detail both actual and projected (through 1985) activities. There are also annual reports only for 1989 and 1991.
